Temporary Disability claim on credit card

Hi, please explain what is needed to submit a claim,what is the turnaround time for application to be assessed, if approved, for how long it would be paid for and what restriction are there with regards to multiple claims on the same account for e.g claim made in 2020 and then again in 2023.
